Preparing practices, clinics and hospitals for the future of value-based care
Documentation, regulatory changes and incentive program criteria shouldn’t stand in the way of delivering quality care and maximizing financial performance. Turn challenges into an advantage with technology that can do the heavy lifting.
As healthcare shifts from fee-for-service to pay-for-performance models, practices that adapt with the change will be well positioned to continue caring for patients and improving their bottom line by taking advantage of value-based payment programs.
CORE is a revolutionary Care Management System that utilizes an interactive web-based delivery system to support physicians and their staff in the art, science, and business of delivering care for medically supervised weight management. It helps coordinate care for obesity-related illness treatment for Medicare patients and others. Developed and supported by an experienced bariatric healthcare and business team, the CORE program brings both clinical and business tools to primary care physicians to ensure best clinical outcomes and practice incomes.
What is unique about the CORE?
The CORE system was created to enable primary care physicians to manage health conditions of patients that are suffering from obesity through the use of innovative technology to enhance individual care, patient relationship management, and improved clinician productivity. The CORE system arms the provider with tools to deliver effective and efficient weight management.
Innovative Communication Platform
The CORE enables two-way communication between the patient and the provider through a web-based system. Traditional Healthcare technologies do not empower the patient to communicate with the provider. It is usually a one-way conversation. Patients to view clinical data, such as labs and x-rays, and update demographics; however, the patient cannot send any clinical data back to the provider.
In the field of weight management, collection of data is much more intensive and critical than the traditional disease management model. Integration of pertinent history directly into the system would significantly reduce the amount of time the clinic staff has to spend keying in data. Upon the patient’s arrival to clinic, the staff will only need to verify/clarify the information. This also allows the provider to be more effective at interviewing the patient since most of the answers are in the system.
Our system helps the provider:
- Effectively deliver lifestyle modification therapy within the time constraints of a busy medical practice.
- Simplify the documentation of all pertinent data points to allow them to capture billable services.
- Deliver lifestyle modification therapy with the help of smart tools and clinical wizards
- Provide better patient care
- Receive better patient feedback
- Encourage patient engagement
- Amplify behavioral change
Advantages of the CORE vs traditional Electronic Systems
|IMPEDIMENTS||Traditional Health IT System||ProNex’s CORE|
|Most PCPs are not adequately trained in weight management therapy.||None||Conveniently delivers effective weight management training to providers through established online education modules.|
|Collection of pertinent nutritional and fitness history through surveys or intensive interviewing process is time consuming.||Office staff still has to manually key this into the System, which is time consuming for the practice.||Empowers two-way communication with patients to facilitate integration of data directly into the system prior to the clinic encounter.|
|Analysis of the patient’ history to make the appropriate diagnosis within the small time frame of most PCP clinic visits||The providers have to assimilated the information and make a diagnosis based upon years of experience in the field of weight management.||Use best practice models to generate diagnosis based upon the patient’ history to guide the PCP’ therapy.|
|Development of an effective treatment plan within the small time frame of most PCP clinic visits.||Providers can only document the treatment plan. There is no assistance from the system.||Based upon best practice models, this platform can generate a “TOP 10” list of nutritional/fitness skills to help the provider deliver more effective therapy. Patients can also walk away from a visit with tangible goals and expectations.|
|Ongoing monitoring and support to ensure accountability is difficult||Does not provide any additional communication with the practice. Patients can only view clinical data.||Continue to hold patient accountable for their actions with meal plans, food/activity logs, and tracking of skills acquisition.|
|Ability to obtain significant outcomes data as it relates to weight management.||Usually lack data fields pertinent to weight management; therefore, special templates have to be created to enable collection of this data.||Pertinent clinical data can be collected through the physician patient communications platform (PPCP) to generate pertinent reports as it relates to weight management.|
|Ongoing patient education and counseling is time consuming and labor intensive.||NONE||Provides ongoing education through an online curriculum designed by experience weight management professionals|
