The Plantrician Project, a global leader in advancing evidence-based nutrition and lifestyle-based care, has announced a groundbreaking partnership with ProNex Inc., the healthcare technology company behind the CORE® LMP platform. This collaboration delivers a powerful digital tool for Lifestyle Medicine Practitioners (LMPs) to effectively prescribe and track the six pillars of lifestyle medicine in clinical practice: whole food plant-based nutrition, physical activity, restorative sleep, stress management, positive social connection, and avoidance of risky substances.
Successful Pilot Leads to National Rollout
Following a successful multi-site pilot program involving certified Plantrician Providers, the CORE® LMP platform demonstrated improvements in patient engagement, clinical workflow efficiency, and the ability to track outcomes across all six pillars. Based on this success, the platform is now being made available to Plantrician-certified practitioners nationwide, effective June 1, 2026.
“We’re excited to bring this transformative solution to our provider network,” said Scott Stoll, MD, Board Chair and Co-Founder of The Plantrician Project. “Our mission has always been to empower practitioners with evidence-based tools, intuitive AI integration, and GLP-1 tracking. CORE® LMP brings that mission to life—bridging education with implementation in a way that is scalable, measurable, and personalized.”
CORE® LMP: The First-of-Its-Kind Digital Workflow for Lifestyle Prescriptions
The CORE® LMP platform is purpose-built for clinicians practicing lifestyle medicine. With intuitive features for personalized care planning, integrated education modules, outcome tracking, and patient communication, CORE® LMP enables practitioners to operationalize what has traditionally been difficult to scale.
The platform allows providers to:
· Prescribe nutrition using the Plantrician Plate model, with personalized meal logging, grocery lists, and culturally relevant plant-based recipes
· Assign movement goals, with automated reminders and progress tracking
· Assess and support stress, sleep, and connection through lifestyle assessment tools and journaling features
· Track reduction in risky behaviors, such as tobacco and alcohol use
· Engage patients with resources, accountability, and real-time messaging
“The partnership with The Plantrician Project represents a pivotal step forward,” said Fred Pira, CEO of ProNex Inc. “With CORE®LMP, we’re equipping clinicians with the tools they need to deliver whole-person care—efficiently, effectively, and at scale.”
Meeting the Moment in Chronic Disease Care
With 80% of chronic disease linked to lifestyle factors, patients and practitioners alike are demanding solutions that go beyond prescriptions. This partnership offers a comprehensive, tech-enabled solution for delivering true root-cause care while aligning with value-based reimbursement trends.
